High Point Awards Nomination Form
Animals MUST be registered with the Welara Registry prior to Nomination for eligibility. Be sure to include all requested information carefully. Complete the Nomination eForm below, and send the required nomination fee of $15 within 21 days thereafter, NO exceptions. Multiple simultaneous nominations of two (2) or more are eligible for a fee discount, please see current rate sheet.

Nomination fee entitles your animal to participate in this program for ONE year, at which time you may choose to renew the nomination for an additional fee.

After you press the SEND FORM button, the information will be recorded. You will be sent an automated eMail to verify the Registry received your request.
Shortly thereafter you will receive from a Welara Registry representative an official letter by eMail verifying Nomination. That eMail also will include an Awards Received form that you MUST print or photocopy, and present to the show official in charge of awards, at each show or event your animal participates in during the show year. The show official is to verify the accuracy of awards you have claimed as won at that show or event, and sign the form.

Then, the registered animals owner (yourself) is to return by Postal Mail or Fax a copy of each and every filled out Awards Received Form to the American Welara Pony Registry at the address OR Fax number listed below. You may either return them to us after each event, OR all at the same time after show season ends. But they MUST be in our office by or before December 31 of the nominated show year. Failure to follow these instructions may terminate Nomination, awards and purse for the current show year.

If you do not own a printer, please contact the Registry and we will send the required form to you by Postal Mail

If for any reason you do not receive the approval verification letter which also contains the form mentioned above, please request it directly from the Registry at the addresses listed below, OR you may download the Awards Received Form from our Forms page available HERE.
You must first be Nominated by completing the form and paying the Nomination fee before the Awards Received Form can be used.
